Job Description
At Commure, we're building the AI Operating System for healthcare, the foundation that defines how care is delivered, documented, and financed. Our platform spans the full care journey: Ambient AI and Dictation eliminating documentation burden at the point of care, intelligent Agents automating patient and revenue workflows, and autonomous RCM processing billions in claims, all on a single AI-native platform integrated with 60+ EHRs.
Healthcare carries a $1 trillion administrative burden and we're at the center of transforming it. Today, 500,000+ clinicians across 500+ healthcare organizations nationwide trust Commure to handle $25B+ in annual claims and support over 200 million patient interactions. Our latest $70M raise at a $7B valuation reflects the confidence the market has placed in this mission. We've also been named to the Fortune Future 50 list and the 2026 AI Breakthrough Awards for “Overall NLP Company of the Year.”
Our team works directly alongside clinicians, not through layers of process, which means the gap between what you build and its impact on patient care is immediate. We move fast, deploy daily, and take full ownership from early thinking to production. If you're energized by hard problems, high stakes, and a team that holds itself to a high bar, you'll find your people here.
The future of healthcare is being built right now. Come deliver this transformation.
About the Role
Commure is hiring a Senior Software Engineer in Rio de Janeiro to help build and scale our RCM Email Agents.
Healthcare revenue cycle teams manage an enormous volume of operational email—from claim and payment questions to enrollment, reconciliation, and customer support workflows. Our agents understand these messages, gather the necessary context, take action across internal systems, and resolve work that would otherwise require significant manual effort.
You’ll own these systems end to end: the agent harness and tools, backend APIs, data models, operator-facing interfaces, evaluations, and production infrastructure. You’ll work closely with engineers and RCM operators to turn complex workflows into reliable agents that can operate safely at scale.
This is a hands-on role for an experienced product engineer who enjoys moving quickly, working across the stack, and owning production outcomes—not just writing isolated services.
What You’ll Do
Design, build, and operate production AI agents that handle high-volume RCM email workflows.
Develop agent harnesses that support tool use, structured workflows, context management, human escalation, and reliable execution.
Build scalable backend services and APIs using Python and FastAPI.
Create intuitive React and JavaScript interfaces for operations teams to review, manage, and improve agent activity.
Design PostgreSQL schemas and data pipelines that support workflow state, auditability, analytics, and agent performance.
Integrate agents with email systems, internal Commure services, healthcare workflows, and third-party platforms.
Build evaluation frameworks to measure agent accuracy, resolution rate, escalation rate, latency, and cost.
Establish monitoring, alerting, reporting, and safeguards for business-critical agent workflows.
Investigate production failures, identify root causes, and improve the reliability of the overall system.
Work directly with RCM operators and customers to understand workflows and rapidly turn their feedback into product improvements.
Own projects from initial problem definition through implementation, deployment, and measurable production impact.
Help establish a strong engineering culture as we grow our team in Rio.
What You Have
Professional experience building and operating production software systems.
Strong proficiency in Python, including experience developing APIs and services with FastAPI.
Strong full-stack development skills, with experience building modern applications using React and JavaScript or TypeScript.
Experience designing data models and production systems with PostgreSQL.
Hands-on experience building with LLMs, AI agents, or agent harnesses, including concepts such as tool calling, orchestration, context management, evaluations, retries, and human-in-the-loop workflows.
A strong understanding of production engineering, including testing, observability, alerting, incident response, and system reliability.
Experience with containers and cloud infrastructure, such as Docker and Kubernetes.
The ability to move quickly while maintaining a high bar for correctness, maintainability, and user experience.
Strong product judgment and an instinct for simplifying complex operational workflows.
Excellent ownership and communication skills, including the ability to work directly with technical and non-technical stakeholders.
A builder mindset and excitement about applying AI to high-impact healthcare problems.
Bachelor’s or Master’s degree in Computer Science, Engineering, or a related field—or equivalent practical experience.
Nice to Have
Experience building email agents, support automation, workflow engines, or other asynchronous processing systems.
Experience developing evaluation systems for LLM-powered products.
Familiarity with revenue cycle management, healthcare billing, claims, remittance, or practice-management workflows.
Experience building systems that require strong auditability, security, and data privacy.
Experience operating high-volume distributed systems with queues, background workers, and third-party integrations.
